Job Title: Senior Support Worker
Staff Support:
- Have the responsibility and resilience to support staff’s confidence and skills in working with people in their own homes.
- Oversee a team of support workers, offering guidance, on-site coaching, and mentoring, for all staff.
- Working with line manager to support and pass new staff during shadowing, providing induction, training, and supervision to staff, Train and mentor staff in safe medication handling and administration and assess competency in safe medication handling and administration.
- Monitor staff adherence to care plans and identify areas of improvement.
- Carrying out spot checks and audits on quality of care, medication administration practices, identifying errors and near-misses, promptly addressing performance concerns and escalating as identified and sharing the reports with line managers.
Maintaining quality and Compliance
- Carrying out initial and ongoing risk assessments, ensuring safe environments for clients and staff Working with line manager to conduct client reviews to ensure care plans remain appropriate and up to date.
- Maintaining strong and reputable relationships with clients and their families and responding to other concerns and care changes.
- Completing accurate information on visits, reviewing notes and incidents logs report to line manager Assist in maintaining accurate and up to date audits reports that cover quality of care, staff practice and training medication disposal records.
- Using spot checks to identify training needs and key policies and procedures to support staff improve practice.
- Reporting all concerns to the office and taking actions to escalate client’s health changes to right professionals Contributing to team meetings and improvements to care and support provision by staff.
- Working closely with Care Coordination to ensure effective rostering of staff paying particular attention to skills and clients’ support needs.
- Providing hands-on care as required to maintain service continuity and starting and ending runs as needed.
Monitor practice and promote high standards of care of clients in line with organisational policies, relevant legislation, and CQC. - Carrying out client medication stock checks, ensuring you check expired, discontinued medication, correctly log on system, and return them to pharmacy.
- Communicate effectively with pharmacies, GPs health care professionals, staff and clients regarding medication changes and concerns.
Attributes to this post
At least 18 months’ experience as care worker with Care Navigation or elsewhere.
Must be a driver with full UK driving licence. Attention to detail, strong communication, leadership skills, and commitment to safe, person-centred care.
Salary: £13.04/hr.
